・Receipt date ・ Name of the person who received the vaccination ・ Amount of receipt ・ Name of medical institution ・ [Influenza vaccination cost]must be clearly stated ・ Medical institution receipt stampmust be entered.・If multiple people are vaccinated at the same time, please receive them separately.
If you cannot issue them separately, ask the medical institution to fill in the [name of the inoculator] and [breakdown of charges].
If there is a national or local government vaccination cost burden (subsidy), the amount after deducting that amount will be covered by the health insurance association.
